Informed Consent Agreement
Before beginning any assessment, users must affirm the following:
- I understand that The Marriage Blue Book™ is a disclosure and discussion tool.
- I understand that it is not a background investigation.
- I understand that disclosures may be incomplete, inaccurate, or unverified.
- I understand that no assessment can guarantee marital success or failure.
- I understand that I remain responsible for all relationship and marriage decisions.
- I understand that assessment results are educational and informational only.
- I voluntarily choose to participate.
Required checkbox: “I have read and agree to the Informed Consent Agreement.”
Partner Invitation Consent
Before sending a partner invitation, users must affirm the following:
- I understand that participation must be voluntary.
- I understand that The Marriage Blue Book™ should not be used to coerce, pressure, threaten, manipulate, or control another person.
- I understand that invited individuals may decline participation.
- I understand that assessment results remain confidential unless voluntarily shared.
Required checkbox: “I agree to the Partner Invitation Consent Statement.”
